COMMUNITY HABILITATION

LOGAN provides services for adults and children whose impairments have delayed or blocked developmental skills, creating a barrier for activities of daily life.

Whether these delays are cognitive, social, fine motor, gross motor, or mobility, our habilitation program will improve these skills and enhance your child’s quality of life.

After assessing his or her needs, LOGAN’s program staff is trained to guide your son or daughter one-on-one, or if it’s right, in a group through a variety of community activities based on their interests. Some enjoy volunteering, others like to attend various community activities. In some cases, we come into a classroom in a college/technical college setting to help them successfully navigate that environment. Computer skills, learning to use an iPad, successfully using public transportation are all areas in which we can help.

And don’t worry, we’ll work with your son or daughter as long as they need to achieve their personal goals.

Financial Support

Our Community Habilitation Services can be funded in a variety of ways including a Family Supports waiver, a Community Integration, and Habilitation waiver, a nursing facility budget, a group living contract, or private pay.
